Today's Incident: Riverside Hospital

Thank you so much for taking over for me.  Who knew today would be such a crazy day?  We had this incident this morning and OSHA was contacted, now I have to visit another site where we just had an injury.  I did most of the preliminary work for you here, but I need you to put it all together and figure out why this happened.  You have 60 minutes to investigate the incident and use the Five Whys to create a Root Cause Analysis.  Good luck!
